📖What it means
An action that contradicts the constitution, or a policy that conflicts with a constitutional principle. A violation must be logged, reported to the admin, and remediated — by fixing the policy, rolling back the action, or both. Code that breaks a rule which is itself correct is a violation: it redirects to the operator and the target is abandoned, never autonomously patched.

A defect is something not working as designed; a violation is something working exactly as built, against the rules.

A finding is what the auditor observed; a violation is the finding once it is confirmed to break a rule.
